| This is the contract each PHO contracts with WIPA to provide it with management services. |
| Capital Trust Deed | |
| This is the legal constitution of each PHO that has been submitted to the Companies Office and the Department of Inland Revenue for full charitable trust status. |
| Capital Business Plan | |
| These plans set out the direction and intention of each PHO. These plans were submitted to the Capital and Coast DHB for approval prior to the PHOs providing services. The plans include outlines of the health promotion initiatives this PHO will be delivering to its registered population during the contract year. These plans also outline the services to improve access initiatives the PHOs will be delivering to its registered population during the contract year. These services are targeted to high deprivation parts of the community, and Maori and Pacific people, therefore, they are not available to all registered patients of the PHO. This is because research shows that these groups have the a lower health status than other people in the country. |
