What is the Otaki Primary Health Organisation?

The Otaki PHO is a not for profit Charitable Trust that provides primary care to people living in the Otaki community.

PHO's are a new approach for managing primary care services for the community. The key roles of the PHO are to:

• Work with local communities and enrolled populations
• Identify and remove health inequalities
• improve, maintain and restore people's health
• Co-ordinate care accross service areas
• Further develop a primary health care workforce
• Development of robust plans for service development
