Maori Health Plan 2008 - 2009
This plan jointly with the Services to Improve Access Plan, Health Promotion Plan and Foe A Te Vaka provide the operational direction for Tumai this period.
Foe A Te Vaka 2008 - 2008 - 2009
This document sets out the Foe A Te Vaka priority initiatives for the July 2008 - June 2009 year. This plan is used to consolidate Pacific development implemented during the past year.
Tumai mo te Iwi Business Plan 2008 - 2010
These plans set out the direction and intention of the PHO. These plans were submitted to the Capital and Cost DHB for approval prior to the PHO providing services. The plans include outlines of the Health Promotion and Services to Improve Access initiatives the PHO will be delivering to it`s registered population during the contract year. These services are targeted to high deprivation parts of the community, Maori and Pacific people, therefore, they are not available to all registered patients of the PHO.
Tumai mo te Iwi Strategic Plan
This Strategic Plan forms an overarching guide to all activities and participation undertaken by Tumai. Dovetailing into this plan are our annual operational Maori Health Implementation Plan, Foe A Te Vaka (Pacific Health plan), Services To Improve Access Plan and the Health Promotion Plan. Tumai is supported in the development of these plans through our Maori, Provider, Pacific and Community Caucus’s, which we work alongside to seek input and mandate, as the key priorities for each year are set.

Tumai mo te Iwi SIA Plan 2008 - 2009
This document sets out the Srvices to Improve Access (SIA) priority initiatives for the July 2008 - June 2009 year. This plan is used to consolidate SIA initiatives implemented over the first three years of Tumai to improve access to health care for high needs patients/clients.
Health Promotion Implementation Plan 08 - 09
This implementation plan outlines the detail of the health promotion initiatives for July 2008 - June 2009. The Tumai Well Health Promoter (FTE), the Physical Activity Educator (.5FTE) and the Tumai HEHA coordinator (.5FTE) continue to provide leadershiop for the implementation of Health Promotion within Tumai and are responsible for the community participation at health related events.
Healthy Eating and Healthy Action Plan June 2008 - July 2009
This Service delivery plan for this HEHA initiative is a combined initiative by Capital and Coast District Health Board, Porirua Health Plus PHO and Tumai mo te Iwi PHO in collaboration with Porirua City Council and the Regional Public Health Service, Wellington Regional Diabetes Trust and Porirua Healthlinks.

Trust Deed Porirua
This is the legal constitution of each PHO that has been submitted to the Companies Office and the Department of Inland Revenue for full charitable trust status.
Porirua PHO - WIPA Management Agreement
This is the contract each PHO contracts with WIPA to provide it with management services.
Wipa
